UnitedHealthcare, Ready Responders Partner with New Orleans East Hospital to Expand At-Home Health Services

Ready Responders and UnitedHealthcare are partnering with New Orleans East Hospital to identify patients who can receive services through in-home settings instead of the emergency room.

This new relationship with New Orleans East Hospital is part of a pilot Ready Responders and UnitedHealthcare announced in April that provides at-home triage care for patients experiencing non-emergent or non-life-threatening medical issues. Louisianans enrolled in UnitedHealthcare employer-sponsored, individual, or Medicaid health plans can now benefit from this new collaboration.

New Orleans East Hospital clinical team members identify patients who are at-risk and have non-life-threatening medical issues, and who are eligible to enroll in the Ready Responders Community Care program that connects participants to local resources that can help them improve their health. The program educates patients on how to effectively navigate and connect with essential in-home and community resources that can identify the root causes for frequent emergency services use, which in turn ultimately increases access to care, and reduces long wait times and costs of emergency services. Through the program’s weekly visits, Ready Responders refers patients to medical and social services that address their current health care gaps and social determinants of health that affect their well-being and quality of life.

Ready Responders is staffed by a team of local emergency medical technicians (EMTs), paramedics, registered nurses, nurse practitioners, physicians, and physician assistants who can be dispatched once a call is placed to the Ready Responders hotline.
